Completing the UWSS COVID-19 Telecommuting Agreement


This document goes over common issues relating to the accessibility of the COVID-19 Telecommuting Agreement for UW-Shared Services staff.

The COVID-19 Telecommuting Agreement is a form covering expectations of working remotely until the end of the pandemic. Once the supervisor approves your completed form, the request is sent to HR to maintain. HR will track which staff members have completed the form and send reminders.

The Telecommuting Agreement is only in effect until stay-at-home restrictions are lifted in the state.

Common Issues

The UW-Shared Services Telecommuting Agreement uses a web-based application called BP Logix. The application needs to allow pop-ups to be enabled. To do so, click the form link, allow pop-ups for Then, you will need to close the browser or browser tab and click on the link a second time.

Please try the following troubleshooting steps if you experience any other issues accessing the form:
1. Clear your browser cookies and cache.
2. Use another browser (Internet Explorer, Firefox, Chrome, Microsoft Edge).
3. Open the link in an incognito or private window.
4. Close the browser you are using, and re-open it.

Enable Pop-Ups

Enabling pop-ups differs depending on the browser you are using. Find your preferred browser below.

Google Chrome:

Mozilla Firefox:

  1. Open browser.
  2. Click on the Menu icon in the top right corner and select Options.
  3. Open Privacy & Security settings. Under Permissions, click on the Exceptions button to the right of Block pop-up windows (shown below).
  4. Enter the following address into the URL box:
  5. Click Allow, then Save Changes.

Internet Explorer:

  1. Open browser.
  2. Click on the Settings cog in the upper right corner. Open Internet Options.
  3. On the Privacy tab, click Settings under Pop-up Blocker (shown below).
  4. Enter the following address into the URL box:
  5. Click Add, then Close.


  1. Open Telecommute Agreement link.
  2. In the address bar, click on the pop-up block notification.
  3. Click Allow for the website.
